The foundation of my hypnosis and wellness practice consists of a trusting and open relationship between my clients and me. Together we agree that:
1. My practice is NOT a traditional mental health counseling or psychiatric care practice. I am functioning as a wellness support practitioner, not as a licensed mental health professional. This practice is not a replacement for traditional mental health counseling, psychiatric interventions, medical or other licensed professional treatment for mental illness, professional medical advice, financial assistance, legal counsel, or other professional services.
2. My practice is geared toward women who want to make behavioral changes in their lives, achieve their goals, and find support through transitions.
3. My practice is designed to address the specific issues and challenges as outlined by the client.
4. My practice is designed to address the needs of each client and there is absolutely no long-term commitment. Our relationship can be terminated by either party at any time.
5. My practice is a partnership and requires an engaged client. Together we will work on issues like goal attainment, behavioral changes, weight loss, life transitions, etc.
6. Both parties will be diligent and prompt for sessions in order to make the most of the time together. We will end on time and if either party must cancel or re-schedule, it will be done so with as much notice as possible.
7. Payment for services will be discussed prior to providing wellness support and both parties will agree to a fee, form of payment, procedures for canceled appointments, and initial length of commitment.
8. Although I am not functioning as a licensed mental health counselor, I agree to abide by the American Counseling Association's strict guidelines for confidentiality. I agree to keep all information strictly confidential, except in those situations where such confidentiality would violate the law.
9. I approach my practice understanding that each client is guided by her core values and beliefs. I honor and respect the client's belief system and support their spiritual journey.
10. You agree that this document represents our mutual understanding of my practice and our relationship.
